How to Access
- Step 1: Bring this form PDF to your healthcare provider to fill out your prescription for exercise
- Step 2: You will be called after your healthcare provider sends the prescription to a Canadian Society for Exercise Physiology-certified exercise physiologist staff to arrange your first exercise appointment.
- Step 3: Try it for free! The first appointment includes an orientation to the facility, brief health and exercise readiness assessments, and helping you get signed up for YMCA membership (including applying for financial assistance if applicable), or if you’re not quite sure, you can “Try the Y” for one week free!
- Step 4: Get a personalized fitness plan with your YMCA membership, and meet with a health coach 4 times a year to revisit and refresh your Exercise is Medicine program.
