Health Services for Community Living (HSCL) - Island Health Community Health Services
Disability Services
Provided by Island Health
HSCL may be appropriate for patients who require support with:
- Health assessment and care planning related to developmental disability and complex health needs
- Education and support for patients, families, caregivers, and support providers to implement health care plans
- Accessing and coordinating community health resources and services
- Nutrition, oral health, rehabilitation, nursing, or communication concerns related to health and function (availability may vary by geographic location)
- Health planning intended to improve outcomes and enhance quality of life
- Patient must be a Canadian citizen, or have permanent resident status
- Patient must be a resident of British Columbia
- HSCL services are available at no cost for patients living with complex health care needs requiring care planning or coordination, who meet the Community Living BC eligibility requirements
- General public can call their local Community Access Centre (see below) to request further information and self-refer.
