Provides support for managing daily challenges or to adjust when illness, injury, or disability affects daily living.
Island Health Community Health Services provides short-term social worker support when health, disability, safety, or life changes affect a person’s well-being, care planning, or ability to live safely at home. Social workers can help with concerns such as:
  • Planning for future care (Advance Care Planning)
  • Financial, housing, or practical concerns related to health
  • Finding community supports and advocating for care needs
  • Support related to palliative care, end-of-life care, and grief
  • Safety concerns, including adult abuse, neglect, or self-neglect (Adult Guardianship Act)

Eligibility depends on health conditions, existing supports, and care needs in addition to:
  • Patient must be a Canadian citizen, or have permanent resident status
  • Patient must be a resident of British Columbia
How to Access
  • Access starts through the Community Access Centres using a central intake and assessment process. During this process, clinicians will assess eligibility, clinical need, function, and safety to determine if Social Work is the right service. Services vary by location.
