Home support is available for individuals requiring assistance with activities of daily living due to health conditions, functional limitations, recovery needs, caregiver support needs, or end-of-life care.
Island Health Community Health Services provides subsidized Home Support for eligible patients who require assistance with activities of daily living due to health conditions, functional limitations, recovery needs, caregiver support needs, or end-of-life care. In order to receive subsidized assistance, a financial assessment will be conducted to determine the ammount subsidized by the Ministry of Health. Services are provided in the patient’s home by Community Health Workers and are based on assessed need, eligibility, safety, and local service availability.

Home Support may be appropriate for patients who require assistance with:
  • Personal care, including bathing, toileting, dressing, grooming, or getting ready for the day or night
  • Medication assistance
  • Heating and serving simple meals
  • Short-term support during illness, post-acute recovery, or after surgery
  • Caregiver respite when personal care or supervision needs can be supported through Home Support
  • Palliative or end-of-life care at home
Please note that Home Support does not provide:
  • Housekeeping, laundry, grocery shopping, errands, or transportation
  • Banking, financial tasks, or bill payment support
  • Care for pets or other household members
  • Tasks requiring heavy lifting, climbing, or services outside the assessed care plan
Eligibility
  • Patient must be a Canadian citizen, or have permanent resident status
  • Patient must be a resident of British Columbia
  • Eligibility depends on health conditions, exisisting supports, and care needs
How to Access
  • Access starts through the Community Access Centres using a central intake and assessment process. During this process, clinicians will assess eligibility, clinical need, function, and safety to determine if Home Care Nursing is the right service. Services vary by location.
