Social Prescribing - Vancouver PCN 3 South: Renfrew Collingwood (Collingwood Neighbourhood House)

Works with seniors to develop and implement a wellness plan based on their needs and goals by connecting them to local supports.
Social Prescribing is a structured way of bridging between health and social services by supporting seniors and older adults in Renfrew-Collingwood with local, non-clinical services to holistically address their needs. Clients work closely with CNH staff to develop and implement a wellness plan based on their needs and goals by connecting them to local supports.

Eligibility Criteria
  • Isolated 55+ seniors living in the Renfrew Collingwood area
  • Experiencing poor physical and mental health
  • Would benefit from greater social inclusion or activity
  • Marginalized or experiencing barriers accessing social or health and wellness programs
  • Making frequent trips to the ER or primary care

One-on-one support provided to access programming such as:
  • Physical activities i.e. walking clubs
  • Nutrition/food security programs
  • Tailored programming built to needs of client
  • Brief Action Planning via telephone and telephone check ins
  • Social programs and services in the community i.e. coffee socials, lunch programs, group outings
Registration is required. Email the Community Connector to set up a phone call to begin the referral process.
