Diabetes Education Centre - North Shore
Caregiver Support and Condition Specific Support
Provided by Vancouver Coastal Health
- Group diabetes and prediabetes education
- Individual counselling appointments
- Insulin therapy
- Support for insulin pumps, continuous and flash glucose monitoring
- Diabetes and Pregnancy
- Pediatric Diabetes Clinic
- Young Adult Clinic (for clients transitioning to adult care) and
- Diabetes Foot Care (fees may apply)
Eligibility: North Shore residents ages 17-23 (and then graduate by 23 years of age to adult services) diagnosed with Type 1 or Type 2 diabetes, prediabetes (impaired fasting glucose or glucose intolerance), diabetes in pregnancy and insulin resistance.
Interpreter services for non-English speaking clients (usually by telephone).
Clients should plan to be at the clinic for approximately 2 hours at their first visit, and 1.5 hours for follow-up visits. As much as possible, we will try to see clients every 3 months.
Before a client's first visit, they will be asked to fill in an online form about their medical history and diabetes self-management.
At follow-up visits, they will be asked to complete a discussion sheet to identify topics they would like to discuss at their appointment. The majority of visits will be done with the client alone. Parents/caregivers are welcome if the client would like them to attend. All communication in the clinic and outside of the clinic will be directly to the patient.
