Seniors Outpatient Clinics (SOPC) - Island Health Area

Seniors Outpatient Clinics work to improve quality of life by optimizing ability and independence through assessments and recommended treatment plans.
The Seniors Outpatient Clinic (SOPC) provides comprehensive assessment and treatment planning for older adults whose medical, cognitive, functional, or psychiatric concerns are impacting daily functioning. SOPC services are delivered by Geriatric Medicine and/or Geriatric Psychiatry physicians, depending on the resources available at each location. SOPCs are supported by an interdisciplinary team, which may include nursing, occupational therapy, physiotherapy, social work, pharmacy, and access to group-based education. SOPCs are currently located in Comox, Nanaimo, Saanichton, and Victoria.
Note: For SOPCs without onsite Geriatric Psychiatry, patients meeting Geriatric Psychiatry criteria may be redirected to a GSS-MHSU outreach team connected to the local Community Health Services office.

Appropriate for patients with:
  • Cognitive impairment or suspected neurocognitive disorder
  • Complex multi system medical issues or geriatric syndromes (falls, frailty, polypharmacy, continence issues, chronic disease complexity)
  • Functional decline, self neglect, or caregiver strain
  • Mood, anxiety, psychotic, or behavioural symptoms emerging in later life
  • Neuropsychiatric symptoms of dementia
  • Cognitive or functional decline in the context of substance use
  • Movement disorders (e.g., Parkinson’s disease) affecting function or independence
Limitation: SOPC does not complete stand alone driving assessments. For concerns about a driving ability, please complete a Report of a Condition Affecting Fitness and Ability to Drive form and send to RoadSafety BC.

How to Access This Service:
  • A referral is required
  • Primary healthcare providers (family or nurse practitioner) and other health professionals will send completed referral form to Community Access using the CHS/GSS referral form (providers see below)
