Community Dietitian Services - Island Health Community Health Services
Food, Healthcare Providers, Home Health Care, and Public Health
Provided by Island Health
- Dysphagia
- Complex therapeutic diets or nutrition support for chronic disease management
- Food allergy, intolerance, celiac disease, or other medically required diet modifications
- Malnutrition risk, unintentional weight loss, poor intake, or nutrition-related functional decline
- Nutrition concerns related to frailty, dementia, mental health, post-acute recovery, or palliative care
- Preventive nutrition counselling when clinical risk factors are present and community dietitian follow-up is appropriate
- Patient must be a Canadian citizen, or have permanent resident status
- Patient must be a resident of British Columbia
- Eligibility depends on health conditions, exisisting supports, and care needs
- Access starts through the Community Access Centres using a central intake and assessment process. During this process, clinicians will assess eligibility, clinical need, function, and safety to determine if Home Care Nursing is the right service. Services vary by location.
