Provides community based respiratory self management education for people with respiratory illness.
Respiratory self management education is offered to people living with a respiratory diagnosis in all of Fraser Health. A Respiratory Therapist will complete a respiratory assessment and provide respiratory self management education to clients in their home virtually and using home health remote monitoring systems. The service involves a team-based approach including the Primary Care Provider and Respiratory Therapist (RT). The RT visits the client in their home to provide coaching of self-management skills related to respiratory conditions including education on the use of prescribed medications, identification of the signs and symptoms and management of flare ups as appropriate. Client goals are set to identify strategies for the management of activities of daily living, diet and exercise as well as improve quality of life.

Clients receive services based on their individual needs. Once a client is able to recognize a flare up and self manage, they are considered to have been successful on this program and discharged with the ability to contact CRS should they feel that they need more support. Client can be re-referred at any time. Community Respiratory Services’ COVID-19 Intensive Monitoring Program has replaced COVID-19 Care at Home Program providing ongoing care and management to eligible COVID-19 patients discharged from hospital.

Eligibility:
  • Client should have a respiratory condition diagnosis.
  • If client is homebound and needs COPD screening, a physician can send a request for screening spirometry.
  • Must reside in the Fraser Health region.
