Clients receive services based on their individual needs. Once a client is able to recognize a flare up and self manage, they are considered to have been successful on this program and discharged with the ability to contact CRS should they feel that they need more support. Client can be re-referred at any time. Community Respiratory Services’ COVID-19 Intensive Monitoring Program has replaced COVID-19 Care at Home Program providing ongoing care and management to eligible COVID-19 patients discharged from hospital.
Eligibility:
- Client should have a respiratory condition diagnosis.
- If client is homebound and needs COPD screening, a physician can send a request for screening spirometry.
- Must reside in the Fraser Health region.
