Seniors' Community Connector - Social Prescribing Program - Richmond

Connects individuals to non-clinical services to help improve their health and wellness.
Richmond’s Community Link social prescribing service helps connect patients to Richmond-based, non-clinical support services throughout the community. Participants of social prescribing programs have reported improvements to their physical and emotional wellbeing, social connections, and ability to thrive.


Referral Details: Family physicians and nurse practitioners are welcome to make referral to our Community Link Worker with the RPCN Community Link Prescription Form.
