Nurses and registered care aides come to a patients' home daily to support them in their recovery journey with a personalized health care plan and medication administration.
The HealthLink Outreach Program provides daily nursing visits to support medication administration and vitals monitoring. The nurse team can administer oral, topical, and certain injectable medications. They monitor many different types of vital signs and health parameters, depending on the patient’s medical conditions and the requests of their prescribers. Patients are typically referred to the program at the time of hospital discharge, or during transitional times when a healthcare provider notices the need for additional community support for medication administration and vitals monitoring. The program's standard of care is one nurse visit per day, and the opportunity for arranging additional visits privately for a fee per visit. This program services the Lower Mainland and Fraser Valley areas. Patients can be referred using HealthLink's online form.

The partnership of HealthLink Outreach and Pharmacity Drugstore results in no charge for daily nurse visits. Prescriptions are filled daily by Pharmacity Drugstore, and patients are responsible for the cost of their daily prescriptions, up to a maximum amount determined by Fair Pharmacare, called the annual family maximum amount (or deductible). The pharmacy offers various services through this program, including blister packing, vaccines, smoking cessation, compounding, and automatic refills.
