Neurological Rehabilitation Outpatient Program

Condition Specific Support, Disability Services, and Home Health Care

An outpatient rehabilitation program for adults with a neurological condition.
The program consists of a multidisciplinary team (OT, PT, SLP, SW, Nurse) that provide outpatient rehab for adults with a neurological condition i.e., CVA. Head Injury, MS, Parkinson's, etc.

The goals of the program are to improve the physical, cognitive and social functioning of adults with neurological conditions, through individual and group treatment. They also enhance the independent function of clients in their homes and community by providing therapeutic support to families and referral to community resources.

To be eligible a patient must:
  • Have a neurological condition.
  • Have the potential to improve his/her level of function, and have realistic goals that can be attained in a timely manner.
  • Require treatment from at least two disciplines. (OT,PT,SW,SLP&RN)
  • Have the tolerance for both the treatment sessions and transportation to and from the program.
  • Be able to be left alone safely or be accompanied by an attendant or family member.
  • Live on the North Shore or in the Coastal Service delivery area.

The Neurological Rehabilitation Outpatient Program currently has a high volume of referrals which has created a lengthy waitlist. Referrals will be seen in the following priorities:
  • Priority 1 (Wait time 1-2 weeks post discharge): Patients being discharged from Lions Gate Hospital.
  • Priority 2 (Wait time 3-4 weeks post discharge): Patients being discharged from other hospitals and are North Shore/Coastal Service Delivery area residents.
  • Priority 3 (Wait time 1-3 months): Patients currently living at home and have a new diagnosis (MS, Stroke, etc.) and have not previously received any intervention. New safety risk.
  • Priority 4 (Wait time of 5+ months): Patients living at home with a chronic condition and may have experienced a recent change in their condition. No immediate safety risk.
