Provides an access point for a wide range of palliative care support services for people living with life-limiting illness and their families
Palliative care support services includes nursing care, home support/personal care supports, care management, rehab therapies, home hospice specialist service, access to residential care hospices or acute care palliative care units.

Referral: Referrals are accepted from any source including the public, family doctors, BC Cancer Agency, outpatients units, and acute care hospitals outside of the Lower Mainland.
