A lifestyle change and support program for individuals living with chronic diseases.
This program offers 1-on-1 appointments as well as group chats on a variety of topics including:
The Chronic Disease Clinic provides education and care to people living with chronic diseases such as heart disease; high cholesterol; diabetes (type I, type II, and gestational; and high blood pressure, as well as people who have had heart attacks.
We believe best health results are achieved when clients are actively involved in managing their illness. We will work with you as you learn about your disease and your concerns.
